With a signed Medical Authorization from the claimant, records can be obtained from medical providers detailing: names of physicians or facilities where claimant has been treated with, date(s) of treatment/prescription, type of visit (emergency room, outpatient, inpatient), prescriptions written, x-rays, medical records, and reasons for treatment or prescriptions.
